- Certified true copy of Offer Letter to CM
- Certified true copy examination result (CM Foundation overall results (completed 2 Semesters)/SPM/ STPM/UEC/A’ Level (ACTUAL RESULTS)
- Certified true copies of evidence of extracurricular activities
